We rely on area treatment facilities and hospitals to help us identify and connect with more children who are battling life-threatening medical conditions.

Qualifications

A child with a life-threatening medical condition, who is younger than 18 years old at the time of the enrollment application, is potentially eligible to enroll with Courageous Kids.

The child’s parent or legal guardian must submit the application online. Printed forms are available for those without Internet access.

Once the application is received, Courageous Kids will begin the enrollment process by contacting the child’s doctor to confirm medical eligibility. Courageous Kids will send a welcome email to the child’s parent or guardian upon confirmation of medical eligibility from the child’s doctor. Then, the enrolled child and family will begin receiving Courageous Kids’ exclusive email invitations to free events and opportunities.
